dc.description.abstractProblemin Tanımı: Multipl Skleroz (MS) merkezi sinir sisteminin inflamatuar bir hastalığıdır. Bilişsel yıkım ve depresyon MS’de en sık görülen psikiyatrik tablolardır. Bilişsel yıkım MS’li hastaların %40-65’ini etkiler ve sıklıkla bellek, dikkat ve bilgi işleme süreçlerinde gözlenir. Dalfampiridin MS tanılı hastalardan da yürüme bozukluklarının semptomatik tedavisinde kullanılmak üzere onay almış farmakolojik bir ajandır. Bu farmakolojik ajanın aynı işleyiş ile bilişsel yıkım tedavisinde kullanılması mümkündür. Bu çalışmanın amacı, dalfampiridin tedavisinin bilişsel fonksiyonlar üzerine etkisini incelemektir. Yöntem: İstanbul Üniversitesi İstanbul Tıp Fakültesi Nöroloji Polikliniği’nde takip edilen ve çalışma kriterlerine uyan MS tanılı ve dalfampiridin başlanması planlanan 12 hasta çalışmaya dahil edildi. Tedavi öncesinde (ön test) ve 10. haftanın sonunda (son test) olmak üzere toplam iki kez belirli nöropsikolojik testler uygulanmıştır. Bilişsel işlevlerin değerlendirmesi için Multipl Skleroz Fonksiyonel Kompozit (MSFC), Çizgi Yönünü Belirleme Testi (ÇYBT), Sözel Akıcılık Testi (WLG), Öktem-Sözel Bellek Süreçleri Testi, Stroop Test (ST) ve Sembol Sayı Modaliteleri Testi (SDMT) kullanıldı. Depresyon CES-Depresyon Ölçeği (CES-D) ve Hastane Anksiyete ve Depresyon Ölçeği (HADÖ) ile ölçüldü. Hastaların tedavi öncesi puanları tedavi sonrası puanları ile karşılaştırıldı. Bulgular: Çalışmaya katılan 12 hastadan 7’si kadın, 5’i erkekti ve yaş ortalaması 42,75±10,58 idi. Dalfampiridin tedavisi alan hastaların 10 hafta sonra SDMT (p=0,005), PASAT 3” (p=0,009), PASAT 2’’ (p=0,033), ÇYBT (p=0,01), WLG (p=0,003), 9 HPT Dominant El Sağ (p=0,005), T25FW (p=0,007) ve Stoop alt testlerinden Stroop II (p=0,002) ve Enterferans (p=0,005) testlerinde anlamlı gelişme gözlenmiştir. Sonuç: Dalfampiridin tedavisinin MS tanılı hastaların yaşadıkları bilişsel bozuklukta olumlu etkilere sahip olabileceği düşünülmüştür. Bu tedavinin MS tanılı hastalar üzerindeki etkisini araştırmak için daha uzun takipli ve daha büyük hasta örneklemlerinde yapılacak çalışmalara ihtiyaç vardır.en_US
dc.description.abstractThe Statement of Problem: Multiple Sclerosis (MS) is an inflammatory disease of the central nervous system. Cognitive impairment and depression are the most common psychiatric symptoms. Cognitive impairment is evident in 40-65% among MS patients and it is most common in memory, attention and information processing functions. Dalfampridine has been approved for the symptomatic treatment of gait disorders in patients with MS. It is possible that dalfampridine could improve the cognitive functions, as well. The aim of this study is to examine the effects of dalfampridine treatment on cognitive functions. Method: Twelve MS patients, who were on follow-up at the Neurology Outpatient Clinic of Istanbul University Istanbul Faculty of Medicine, were recruited for the study. All were planned to start on dalfampridine and were applied the following neuropsychological tests at baseline (pretest) and at the 10th week (posttest) of the treatment. The cognitive functions were assessed using the Multiple Sclerosis Functional Composite (MSFC), Benton Judgement of Line Orientation Test (JLO), Word List Generation (WLG), Auditory Verbal Learning Tedst, Stroop Color Word Interference Test (ST), Symbol Digit Modalities Test (SDMT). The Center of Epidemiologic Studies Depression Scale (CES-D) and the Hospital Anxiety and Depression Scale (HADS) were also applied. All pre- and post- treatment meausures were compared. Results: The mean age of the sample (7 females, 5 males) was 42,75±10,58. At the 10th week of treatment significant improvements were observed at the SDMT (p=0,005), PASAT 3” (p=0,009), PASAT 2’’ (p=0,033), JLO (p=0,01), WLG (p=0,003), 9 HPT Dominant hand right (p=0,005), T25FW (p=0,007) and two subcategories of Stroop II (p=0,002), Interference (p=0,005). Conclusion: Dalfampridine shows promising results in improving cognition in MS patients. Future studies with larger samples and longer follow-ups are needed in order to evaluate the effects of dalfampridine treatment on cognitive functions in MS patients.en_US
