OKB tanısı olan çocuk ve ergenlerde belirti dağılımının ebeveynlerdeki Obsesif Kompülsif Belirti dağılımı ve ebeveyn tutumları ile olan ilişkisi

Abstract

Problemin tanımı: Obsesif Kompulsif Bozukluk (OKB), çocuk ve ergenleri bilişsel, duygusal ve davranışsal alanlarda olumsuz etkileyen ruhsal bir bozukluktur. Çocuk ve ergendeki mevcut obsesif kompulsif semptomatoloji, bu hastaların ebeveynlerindeki obsesifkompulsif semptomlar ve işlevsel olmayan ebeveyn tutumlarından da etkilenebilmektedir. Bu araştırma, OKB tanısı konulan çocuk ve ergenlerin obsesif-kompulsif belirtileri ile ebeveynlerinin obsesif kompulsif belirtileri ve anne baba tutumları arasında ilişki olabileceği hipotezinden yola çıkılarak planlanmıştır. Yöntem: Bu doğrultuda OKB tanısı konulan 7-18 yaşları arasında 18 çocuk ve ergen hasta ve onların ebeveynleri araştırma grubuna, daha önce OKB tanısı almamış almamış 7-18 yaşları arasında 18 çocuk ve ergen ve onların ebeveynleri ise kontrol grubuna dahil edilmiştir. Bu araştırmada DY-BOCS (Boyutsal Yale-Brown Obsesif-Kompulsif Bozukluk Ölçeği) ölçeği ve Padua Envanteri, Anne-Baba Tutum Envanteri, Y-BOCS ölçeği Padua Envanteri, KSADS-PL (Okul Çağı Çocukları İçin Duygulanım Bozuklukları ve Şizofreni Görüşme Çizelgesi-Şimdi ve Yaşam Boyu Şekli) uygulanmıştır. Bulgular: Araştırma sonuçlarına göre OKB tanısı olan ve olmayan gruplar ayrı ayrı değerlendirildiğinde anne-baba tutumu ile çocuk ve ergenlerin obsesif-kompulsif semptomlarının toplam şiddetleri arasında anlamlı bir ilişki saptanmamıştır. Bütün katılımcıların sonuçları birlikte değerlendirildiğinde ebeveynlerinin demokratik tutumu ile OKB toplam ve global şiddet puanları arasında negatif yönde anlamlı bir ilişki saptanmıştır. OKB tanılı çalışma grubunda ebeveyn tutumu ve çocuk ve ergenlerin obsesif-kompulsif semptom boyutları arasında anlamlı bir ilişki bulunmamıştır. OKB tanısı olmayan karşılaştırma grubunda ebeveynin demokratik tutumu ile sıralama, simetri, sayma, tekrar tekrar yapma, her şeyin tam ayarında olması ihtiyacı alt boyutu arasında negatif yönde anlamlı bir ilişki saptanmıştır. Bütün çocuk ve ergenlerin zarar hakkındaki obsesyon ve kompulsiyonları ile ebeveynlerin otoriter ve koruyucu-istekçi tutumları arasında ise pozitif yönde anlamlı bir ilişki bulunmuştur. Sonuç: Kısıtlılıklarına bulunmasına rağmen bu çalışma sonucuna göre demokratik ebeveyn tutumunun OKB’nin semptom şiddetinde koruyucu rol oynadığı öne sürülebilir.

The statement of problem: Obsessive Compulsive Disorder (OCD) is a psychiatric disorder which involves obsessions and compulsions that negatively affect children and adolescents in emotional, cognitive and behavioral areas. The existing OCD symptomatology in children and adolescents may be affected by their parents’ obsessive-compulsive symptoms and dysfunctional parental attitudes. This study relies on the hypothesis that there might be a relationship between obsessive-compulsive symptoms of children and their parents’ obsessive-compulsive symptoms and parental attitudes. Method: Eighteen children and adolescents (aged between 7-18 years) diagnosed with OCD and their parents are recruited to research group while eighteen children and adolescents (aged between 7-18 years) and who are not diagnosed with OCD and their parents are included in control group. DY-BOCS (Dimensional Yale-Brown Obsessive Compulsive Scale), Padua Inventory, Parental Attitude Scale, Y-BOCS Scale Padua Inventory and K-SADS-PL Scale (Schedule for Affective Disorders and Schizophrenia for School-Age Children-Present and Lifetime Version) are used in this study. Results: When the groups are analyzed separately, the results show that there is not a significant relationship between family attitudes and total severity of OCD symptoms in children and adolescents. On the other hand there is a significant negative relationship between authoritative attitude of the parents and total OCD severity of the children and adolescent when both groups are evaluated together. No significant relationship is found between the family attitudes and obsessive-compulsive sub-dimensions in the research group. In the control group, there is a significant negative relationship between the authoritative attitude of the parents and symmetry, ordering, counting, repetitive behavior and perfectionism sub-dimensions of the OCD. There is a positive significant relationship between harm-related obsessions-compulsions of children and adolescents and authoritarian and over protective family attitude when both groups are evaluated together. Conclusion: Despite the limitations of the study, it can be suggested that authoritative attitude of parents seems to be a protective factor in the development of obsessive-compulsive symptom severity of children and adolescents.
