A simple decision support system for diagnosis of pituitary tumors in endocrinology

Abstract

We can face with misdiagnosis and treatment in every field of Medical Sciences. This situation affects directly human life. Since the medical treatment expenses is a heavy burden for the families in Turkey (Gross National Product per capita was {dollar}3,383 in 2003), this leads some tricks in social security system and consequently results in lost of time and money. Therefore, the above explanations encourage us to use expert systems and Information Technologies (IT) in Medical Sciences. For example, usage of Decision Making Techniques and Information Technologies can provide computerized guidance for patients and help doctors to make the diagnose phase shorter and easier which in turn this affects the patients physical & psychological state positively and the cost of diagnosis & treatment. Medical Informatics is the combination of Medical Sciences, Information Technologies and some other disciplines. However, there is a misunderstanding in this concept. In Turkey, usually, Medical Informatics has been understood as 2Hospital Management Systems3 for the appointments, billing and administration. Intelligent use IT is not yet available for the pre-diagnosis of standard sickness. In this study, a prediagnosis software package (S/W) is developed for Endocrinology and Metabolism sicknesses, especially for Acromegaly, Cushing and Prolactinoma, employing the decision making techniques on PHP and MySQL in the form of a web-site. The aim of this web-site is to help visitors to test their existing symptoms. These tests based on the statistics taken from the studies of Endocrinology, Metabolism and Nutritional Diseases Division of Istanbul University.

Yanlış teşhis ve tedavi tıbbın her alanında karşımıza çıkabilmektedir. İnsan sağlığı, psikolojisi ve kişi başına düşen millli gelirin 2003 yılı itibarı ile {dollar}3,383 seviyesinde oluşu gözönüne alındığında, ailelerin bütçesini de etkilemekte olan bu durum; sosyal güvenlik sisteminde ve özel sigorta sektöründe suistimallere sebep olmakta ve ekonomiye de zarar vermektedir. Teşhis ve tedavi ile ilgili alınabilecek yanlış kararlar direk insan hayatını etkilemekte olduğundan, Tıp ve Bilgi Teknolojilerinin ortak çalışma alanları yaratmaları ve geliştirmeleri gereksinimi ortaya çıkmaktadır. Bilgi teknolojileri ve karar verme sistemlerinin kullanımı teşhis sürecini kısaltarak ve hekimlere teşhis koymada yardımcı olarak hem hekime hem de hastaya zaman (fiziksel - psikolojik sağlık ve işgücü açısından) kazandıracaktır. Dünyada Tıp Bilişimi, Tıp, bilişim teknolojileri ve farklı disiplinlerinin kesiştiği bir yerde bulunmaktadır. Ülkemizde ise tıp bilişimi genellikle 2Hastane Bilgi Sistemi Yazılımı3 adıyla, neredeyse tamamı idari amaçlı olan yazılımlardan oluşmaktadır. Kurumlararası, standart yapıda elektronik bilgi değişimi ve paylaşımı yok denecek kadar azdır. Bu çalışmada, PHP ve MySQL kullanılarak bazı hastalıklar için web tabanlı bir ön tanı sistemi geliştirilmiş, bu yolla ziyaretçilerin kendilerini test etmeleri sağlanmaya çalışılmıştır. Her test sonucu tüm detayları ile veritabanında saklanmakta, ziyaretçiye her test yapışında daha önceki test sonuçları tarih ve saat detayı ile sunulmaktadır. Test, İstanbul Üniversitesi Tıp Fakültesi, Endokrinoloji Metabolizma ve Beslenme Bilim Dalı çalışmalarından alınan istatistiklere dayanmaktadır. Hastalıklarla ilgili verilerin iyi organize edilmiş ve sınıflandırılmış olması nedeniyle, özellikle endokrinoloji ve metabolizma hastalıklarından Akromegali, Cushing ve Prolaktinoma üzerinde durulmuştur.
